PENDAFTARAN/REGISTRATION

Pendaftaran peserta secara Online dibuka sekarang hingga 10 Mac 2016/Online Registration is now open until 10 March 2016.

 Sila kemukakan maklumat berikut/Please forward the following informatiom:

1. Nama penuh mengikut kad pengenalan/pasport/Full name       as in ID/pasport;

2. No kad pengenalan/paspot/ID/pasport No;

3. Tarikh lahir/Date of birth;

4. Kategori/Category;

5. No telefon & nama untuk dihubungi bila kecemasan/ Telephone & contact name in case of emergency.

BAYARAN sebanyak RM60.00 bagi seorang peserta hendaklah dimasuk kedalam akaun Rezada Flora @ CIMB Bank No. 8600791926 apabila memohon pendaftaran/To register a PAYMENT of RM60.00 per  participant should be deposited into the account of Rezada Flora @ CIMB Bank No. 8600791926.

Semua maklumat diatas serta salinan transaksi bank hendaklah dihantar dengan whatsapp  ke  019 2628759/All details as required above together with copy of bank transaction should be sent by whatsapp to 019 2628759.
